Attach a copy of the bond and/or proof of financial responsibility obtained by the permit requester In accordance with the UHFMO Fireworks/Pyrotechnics Display Standard section 4.5* Required

Attach a copy of the company's state permit In accordance with the UHFMO Fireworks/Pyrotechnics Display Standard section 4.6* Required

Attach a copy of the shooter’s pyrotechnic operator license, pyrotechnic special effects license, or flames effects operators license in accordance with the UHFMO Fireworks/Pyrotechnics Display Standard section 4.7* Required

Per the UHFMO Fireworks/Pyrotechnics Display Standard section 4.4 attach a detailed diagram of the grounds on which the display is to be held. This shall include all buildings, roads, lines of communication, utility lines, trees, and other overhead obstructions in the surrounding area. If indoor, attach a diagram of the indoor area, any stages, launch locations, and distance between audience and display.* Required
